Heritage Self-determined Option (SDO)
This form is to be completed if the subscriber wishes to transfer to the Self-Determined Option. This option is suitable for beneficiaries who will be enrolling in a post- secondary program that is less than two years or for beneficiaries who will not be attending post-secondary studies.
